We would like to introduce our new Outpatient program. We are excited to announce the Crossroads Aftercare Program (CAP). CAP is now available in the Greater Phoenix Area for both male and female clients seeking outpatient substance abuse treatment.

CAP is an outpatient level of care that provides early intervention to anyone seeking treatment for substance use issues as well as continued care for those who have completed a residential program. CAP is for voluntary clients as well as those who are mandated to treatment by state or federal probation.

 

At this time, all services are done remotely via zoom; group sessions, individual sessions, and case management. Group sessions range from 1-3 times weekly for 3 hours per group. Individual sessions are offered weekly or biweekly, and coordination of care 3-way meetings with the client, case manager, & probation officer are offered monthly, if necessary.

This program is an excellent way to expand the impact Crossroads has on the Phoenix community. We would greatly appreciate all of your help getting the word out to the community about this fantastic opportunity and effective service.
