What Happens After Rehab?

Recovery from SUD does not end when treatment does. Aftercare is just as important as starting treatment. The transition from treatment back into the real world can be difficult, and managing to keep up with your recovery program can be difficult when everyday stressors can trigger anxiety.

At Crossroads, we will help you build an aftercare plan that sets you up for success by providing the tools and resources you need to focus on your recovery.

Recovery is not a linear process. A return to substance use after treatment is common and is a natural part of the recovery process. We open our facilities to help anyone who returns to substance use after treatment and offer them continuous support and high-quality care.

Maintaining a routine and staying engaged in a personal recovery program can be challenging after leaving a Crossroads facility. External pressures and influences may arise, and feelings of anxiety or stress can sometimes become overwhelming.
In aftercare, peer support is available through both telehealth and in-person options, during residential treatment and continuing afterward. Ongoing relapse prevention training and skill-building are also provided to support sustainable recovery.
Continued care through aftercare programs helps individuals stay connected and focused in recovery. When you are provided with accountability, ongoing support, access to resources, and continued skill-building, your recovery doesn’t have to feel constrained to treatment.

Crossroads’ Right Track program is open to individuals who have chosen to pursue recovery, as well as those who have experienced a return to use. You will be connected with medical and mental health support networks, and we will collaborate with you to develop an aftercare plan that aligns with your level of care and ongoing needs
