Please call to set up an appointment if you wish to pick up your records in person. Records will not be provided without an appointment.
Letters: For all letters of enrollment, also known as letter of residency, we ask to make your request one week in advance prior to the date needed- there are no exceptions.
Please be advised: All medical records request take approximately 10 business days from the date the request was received, not including weekends or observed holidays. If the records are needed by a specific date, we ask that a process by date is included on the request. Be advised under HIPAA, Crossroads has 30 calendar days from the date your request was received to comply.
All Authorization forms to Use and Disclose Protected Health Information may be returned via Fax, Mail, or hand delivered.
